CTC community, I need some help. I received three parcels today 14Mar2019, all three are LT11's. One each for my wife and I for TY2017 and one for in my name for TY's 2015, 2016. According to a FOIA request there are no Form 12775 or 8278 in existence for TY 2015 or 2016 in my or my wife's name. I was also told over the phone last year that the CIVPEN was suspended. Any help on how to move forward is greatly appreciated. Sincerely, Mike Gangestad. Liberty or death!

Mike,
I found some information that might be of use to you on losthorizons. I'm guessing some of the form numbers have evolved over the years. I believe the LT11 is the intent to levy and these 2 links might give some insight:

Yes, search LH for Levy information and 6331.

What is the LT11 for? Frivolous Penalties or otherwise?

Yes, CIVPEN $16,286.20 for TY's 2015-16 and $103.71 for 2017

I'm certainly no expert, but based on what George said in the other recent thread, you need to exhaust all administrative procedures first, and then finally, take them to tax court to get a judgement in your favor. Did you file a Collection Due Process? That is my next step. https://www.irs.gov/appeals/collection-due-process-cdp
